The Medical Records Clerk will manage and maintain patient medical records in a correctional healthcare setting, ensuring accuracy, confidentiality, and compliance with healthcare regulations. This role will organize, file, and retrieve medical documents, process documentation requests, and support healthcare staff in maintaining up-to-date patient information. The Medical Records Clerk will also ensure adherence to facility policies and state regulations regarding record-keeping and data privacy.How YOU can Make an Impact:
Create and maintain accurate medical records for inmates, including gathering and organizing necessary documentation.
Collect patient demographic information from various sources, including institutional records and sending facilities.
Initiate health records for new patients and ensure proper filing and accountability of all health records.
Ensure the availability of records by following chart mark-off procedures and facilitating chart location activities.
Review and maintain medical records to ensure completeness, accuracy, and compliance with MEDIKO, facility, and regulatory standards (HIPAA, ACA, NCCHC).
Pull and prepare health records for medical practitioners, including charts for Medical, Dental, Mental Health, Sick Call, Chronic Care, and Transfers.
Maintain active records in alphabetical order and ensure proper processing of diagnostic results, x-rays, MARs, and lab tests.
Comply with all relevant confidentiality standards, including HIPAA, and facility-specific guidelines regarding the release of medical information.
Facilitate the Release of Information (ROI) process from external providers, law enforcement, and authorized individuals.
Ensure all medical records for transfers and legal requests are copied and forwarded in accordance with policy and regulations.
Assist in ordering and maintaining an adequate supply of medical records forms and clerical supplies.
Participate in Quality Improvement activities, cost containment efforts, and maintaining facility safety standards.
Communicate effectively with medical staff, management, and correctional facility staff regarding medical record status and needs.
Maintain and utilize the Electronic Medical Record (EMR) system for scanning and updating patient information.
Assist with departmental tasks and provide support to team members as needed; may require overtime to meet operational needs.
Perform other related duties as assigned.
What we Ask of YOU:
Bachelor's degree in a relevant field (e.g., Human Resources, Finance, Sales, Business Administration, Healthcare Administration, or related discipline) from an accredited institution preferred.
Additional certifications or specialized training may be required depending on the function (e.g., Certified Human Resources Professional (PHR) for HR roles, Certified Public Accountant (CPA) for Finance roles, etc.).
Minimum of 2-3 years of relevant experience in the specific functional area (e.g., HR, Finance, Sales, Marketing, Operations, etc.).
Experience in healthcare, correctional healthcare, or a regulated industry is preferred.
Demonstrated success in applying function-specific skills (e.g., employee relations for HR, financial reporting for Finance, or sales strategy for Sales) is required.
Knowledge of relevant industry-specific regulations, compliance standards, and best practices may be required, particularly for roles in highly regulated functions like HR and Finance.
Relevant industry certifications based on the role (e.g., PHR, SHRM-CP, CPA, etc.).
Active CPR/BLS certificate required (American Heart Association/American Red Cross guideline approved), if role is located in a correctional facility.
